Become a Network Provider
To be eligible to join Anthem as a provider, you must:
- Have an active Department of Health Care Financing and Policy (DHCFP) Medicaid provider ID number.
- Correctly and completely submit the credentialing application and required documentation.
- Be fully credentialed by Anthem.
- Sign and return all contract documents, as applicable. New groups/providers will receive a Participating Health Provider Agreement (contract). Providers joining an existing contracted group must complete the applicable contract documents to be added to the existing contract.
- Upon completion of credentialing and full execution of contract documents, the provider will receive notice from the Anthem Network department with the effective date of participation, along with the fully executed contract (if it is a new contract). Services rendered prior to this effective date will be processed as out-of-network.
Please be prepared to provide the following information when joining our network:
- Active Medicaid ID number
- Medicare ID number (if applicable)
- Geographic location(s)
- Provider specialty and services/facility
- Point of contact (name, phone and email)
